Childhood Immunization Clinics are held at the Portage County Health Department
Nursing Clinic on the first floor of the Portage County Administration Building across
from the WIC Office on Chestnut Street entrance.  Clinics are held every Wednesday
from 8:00 a.m. through 12:00 Noon and are Walk-in, no appointment needed.  Infants
through age 21 may use the Childhood Immunization Clinic.

Adult and Travel Immunizations and TB Testing are by Appointment Only
and may be scheduled through Nursing Services at 330-298-4490.
Immunizations are given at the Portage County Health Department Nursing Clinic
across from the WIC Office on Chestnut Street entrance.

Consultation Fee Per Person $50.00
  Per Additional family member.  Not to exceed
$100.00 when family is all seen during the same consultation appointment.		 $25.00


The fee covers your personal meeting with a public health nurse.  The nurse will look up what vaccinations you need and discuss them with you.  If needed, a prescription for malaria medication is included in the consultation fee.
